NICE-SUGAR

Design International RCT, n=6,104
Published 2009 (NEJM)
Result Intensive glucose control (4.5–6.0 mmol/L) increased 90-day mortality (27.5% vs 24.9%) compared to conventional control (<10 mmol/L), with significantly more severe hypoglycaemia.
Practice impact Changed practice globally; standard ICU glucose target is now 6–10 mmol/L. Tight glycaemic control is no longer recommended.
